Improving the health and well-being of patients in the Wisbech area
Primary Care Networks, or PCNs for short, are a key part of the NHS long term plan which involves local practices collaborating together to improve the physical health, mental health and well-being of it's patients. These local networks will provide the structure and funding to create local initiatives and services based on it's patients health needs which have been established through rigorous research. Networks will comprise of a variety of qualified staff including GPs, clinical pharmacists, district nurses, social prescribers, dementia workers and volunteers.
The Wisbech PCN covers the patients within the Wisbech area and the surrounding villages. If you are registered with one of the following practices then you are a patient of the network:

Our aim is simple: to improve the health and well-being of patients within the network. To achieve this we will work together and share resources in order to fulfill the health needs for our patients.
Additional work will be done to ensure that suitable services have been created and that new, more effective models of providing care are integrated across the network.
The PCN is made up of its member practices; all practices will work collaboratively and have a clear idea of what to expect from the PCN and what the responsibilities of each practice are to ensure the best health care for our registered patients with the resources available.
As more funding becomes available for Primary Care through PCNs it is inevitable that the different practices will need to adopt joint collaborative working in many areas. The different practices will have a different culture, ethos and long standing different ways of providing care to their patients; these will be respected allowing patients a choice of practice to register with.
The PCN will continuously strive to explore new ways of working, to provide excellent, efficient and timely care to its registered patients with the resources available.
The PCN will learn from its constituent practices and other organisations to provide excellent care to it's registered patients. In addition it will share any new models of care developed with other PCNs and Primary Care organisations.
